Billing Policy

We accept several types of payment options for our testing services. To discuss other billing options, please contact us.

Institutional Bill

We encourage clients to establish accounts for institutional billing of testing services, however requisitions completed with institutional billing information and no account set up prior to submission are also acceptable. Volume discounts for accounts may be available; please call us at 844-ALLELE2 (255-3532) to inquire.

Self-Pay

If testing is desired but institutional billing is not available, or if self-pay is preferred, patients may pay directly for testing. To do so, they simply need to complete our credit card authorization form.

Payment Options:

• By phone: The patient may call Allele directly at 844-255-3532 to provide credit card information.
• By fax: The patient may complete the credit card form and have their provider fax it to Allele at 509-232-5779.
• With requisition: The patient may complete the credit card form, and the provider can submit it along with the sample and test requisition form.

Please note that tests ordered with the patient-pay billing option will not be processed until payment information is received and a hold is successfully placed on the card for the full amount. The card will be charged once testing is complete. A receipt with CPT codes will be provided if the patient supplies an email or mailing address.

We accept credit, debit, and HSA cards from Visa, Mastercard, Discover, and American Express.

International Specimens

Samples from outside the United States and Canada must be accompanied by full payment at the time of sample submission. Payment may be made by providing a credit card number at the time of sample submission. Please contact us for more information.
